Capital Home Care for Seniors offers customized care options that are as varied and unique as your own needs. And we build as much flexibility into our service as we can. So if and when your schedule changes, ours can too.

 

How do we do that? By taking the time upfront to get to know you and your family. One of our Care Coordinators will conduct a thorough assessment (in your home, free of charge) of your situation, your needs, and your preferences. Then, together we will develop a Plan of Care that is one of a kind, just like you.

Geriatric Care Management

 

If at some point in the process, you feel that your loved one is in need of more comprehensive services than basic non-medical care, Capital Home Care for Seniors offers professional service coordination through our Geriatric Care Management program. This option is especially appealing to families who are responsible for the care of an aging loved one but who, due to time constraints or geography, are not able to provide the necessary hands-on management.

Our Geriatric Care Assessment is a comprehensive tool that allows us to address your loved one's physical, social, and financial needs, as well as conduct a complete home safety review.
Our Geriatric Care Manager can serve as a physical representative for your loved one when you are unable to do so. This allows an advocate to be present to ask questions, document answers, and provide the family with the information in an organized, written format for recordkeeping purposes.
As part of our assessment, our Geriatric Care Manager (GCM) will complete a thorough examination of all living areas within and outside of the home to determine areas of concern. Following the home evaluation, the GCM will provide recommendations to correct any hazardous conditions as part of a written Geriatric Care Plan.
Our Geriatric Care Manager will keep documentation of correspondence with all representatives involved in your loved one's care, including medical professionals, home health care providers, social workers, and discharge planners.
By organizing paperwork, paying bills on time, and keeping appropriate records on file, this service will enable your loved one to keep their household in proper working order and will ensure accurate record keeping for tax preparation.
Our Geriatric Care Manager (GCM) can handle all arrangements for home maintenance and repairs, lawn maintenance, house cleaning, and/or pet care. The GCM can recommend reputable local vendors for these services based on our history of working and residing within your loved one's community.
Our recommended Plan of Care will adhere to all dietary restrictions, and we will provide oversight for proper food rotation and replenishment. We will work with caregivers to ensure that they are aware of any fitness requirements (such as Range of Motion exercises) and/or limitations established in your Plan of Care. If necessary, we can refer your loved one to our LifeFit program, where he/she can work one on one with a degreed Exercise Physiologist so that any post-therapy needs are properly met.
Our Geriatric Care Manager can accompany your loved one to medical appointments, arrange for any necessary medical equipment, and coordinate and manage all home health providers involved in your loved one's care.
Our Geriatric Care Manager can research assisted living homes, rehab hospitals, and nursing facilities, arrange for tours, and can even accompany your loved one if you are unable to attend initial meetings. We can also simplify the complex questions of funding and eligibility.
We have an extensive network of financial planners, elder law attorneys, and insurance professionals who we can rely on for prompt answers to your questions, and we are happy to refer you to any of these established professionals.
When an emergency occurs, we can be reached immediately via our pager system to coordinate care services, or to be with your loved one until the arrival of a family member. Having a local resource present can alleviate feelings of helplessness for family members, and can ensure that care is in place immediately.


Comprehensive Senior Services

 

Capital Home Care for Seniors provides added value through several proprietary services that complement your overall plan of care. In addition to the In-Home Care and Geriatric Care Management services we offer, Capital Home Care for Seniors provides you, your loved one, and your family access to the following wrap-around services:

 

This on-site facility lets families try out assistive listening devices, blind/low vision aids, daily living aids, and more, free of charge. A certified Assistive Technology Specialist is on hand to answer questions and provide demonstrations, enabling families to make informed decisions before purchasing equipment.
Provides restored and refurbished home medical equipment and assistive technology devices to Pennsylvania residents who are unable to obtain the necessary equipment through any other means. If your loved one's insurance does not cover certain equipment, or if you have an immediate need but must wait out a grace period before coverage commences, Changing Hands can help... free of charge.
This fitness and nutrition program has been adapted to address specific aging-related challenges, especially post-rehab and range of motion issues. Individual sessions and group classes are led by degreed Exercise Physiologists at the location of your choice (home, church, fitness facility, etc.).
